TAY – Transition 101

Learning Outcomes: “It’s NOT just about the bed and the paint, or a wing and a prayer ”This workshop is designed to provide both Managers and Direct Support Professionals, with a proactive understanding of the transition process for the TAY and other individuals moving into support within their agency. All of us strive for continuity of care and best practices to ensure consistent, appropriate, and successful transitions. This workshop will provide you with information and guidance to effectively move individuals into your agency, whether from the children’s sector or a family home.
